LCSW Licensed Clinical Social Worker - Inpatient Rehab Unit - days
Full time (80 hours biweekly)
Weekdays, daytime hours; 8 am to 4:30 pm and some flexible evening
Program Description
WellSpan Surgery & Rehabilitation Hospital, on the Apple Hill Medical Campus, is a 73- bed specialty hospital which includes a 48-bed inpatient rehabilitation unit. Patients are typically admitted for 14 days for a wide variety of medically complex rehabilitation needs. Patients participate in intensive rehabilitation services during their stay. Many patients benefit from behavioral health services to help with adjusting, coping strategies, mental health conditions and family/caregiver support. This position complements a team of rehabilitation psychologists to meet the needs of patients and families. LCSW is part of a large interdisciplinary team working together to transition patients back to their homes and community. This Licensed Clinical Social Worker is integral in assisting with behavioral service needs upon discharge.
Job Description:
Under general supervision and direction from the Rehabilitation Psychologists, the LCSW provides individual, group and/or family therapy within the hospital. As directed by Rehabilitation Psychologist, interviews clients and/or families to obtains additional information and provides therapeutic intervention. Collaborates with the treatment team to develop a treatment plan and coordinates discharge planning.
Duties and Responsibilities
- Demonstrate knowledge of psychopathology and competency to apply evidence-based interventions appropriate for the diagnosis.
- Provides therapeutic intervention with client and family including individual, family and group therapies.
- Assesses mood and adjustment and implements interventions and makes recommendations.
- Interviews clients and family members to obtain pertinent information to assist provision of interventions to the patient and/or family member.
- Coordinates care of assigned clients including participation in meetings if requested.
- Prepares and maintains written documentation as required by the profession and the department, including accurate observations of care to the client.
- Submits charges for billing in accordance with standard practices.
- Works collaboratively with psychologists and interdisciplinary team to provide appropriate therapeutic interventions.
- Participates in weekly patient care conferences to address barriers, suggests strategies and guides the interdisciplinary team in the behavioral health plan of care under the direction from the psychologist.
- Works collaboratively with behavioral health team to formalize and finalize an aftercare plan for clients and ensures smooth hand-offs for care coordination.
Qualifications
- Minimum Experience:
- 6 – 12 months
- Minimum Education:
- Master’s Degree
- Required Certification:
- License in PA: Clinical Social Worker (LCSW)
