The transition from Student Nurse to Registered Nurse is a difficult one and mentorship is a key component within the WellSpan Nurse Residency Program.  All Graduate Nurses will be enrolled in this year-long program.  Our curriculum is validated and meets the standards of excellence required by the American Nurses Credentialing Center.  The content is focused on defining clinical practice as well as psychosocial support of the new graduate nurse along with focus on the enculteration of the graduate nurse within the organization.  Unique to WellSpan is the incorporation of 1:1 visitation with the Nurse Resident by a Professional Development Specialist which ensures a smooth transition for our Residents.  WellSpan is the one of the only PTAP (Practice Transition Accreditation Program) in South-Central PA.

N3A is a 30 bed medical telemetry unit as well as a 9 bed observation unit.  We see general medical patients with diagnosis that include CHF, pneumonia, cellulitis, GI Bleeds, Sepsis, alcohol with drawl, drug overdoses, and UTI.  Our observation patients’ most frequent diagnosis include chest pain, rule out stroke, asthma exacerbation, syncope, rehydration and mild DKA.

 

4 Main at York Hospital is a 23 bed Medical Telemetry unit consisting of 9 private rooms and 7 semi-private rooms, serving patients 18 years and older with a variety of medical diagnoses including, but not necessarily limited to, pneumonia, asthma/bronchitis, COPD, pulmonary embolism, diabetes, electrolyte imbalance, GI bleed, stable angina, congestive heart failure, stroke, substance abuse withdrawal, altered mental status and falls.

 

4 Southwest is a fast paced 56 bed, intermediate care cardiopulmonary telemetry unit, serving the adult population with medically acute diagnosis.  We are the only non-ICU unit in the hospital with the capability of caring for patients who are mechanically vented. We are the unit of choice for heart failure, sepsis, respiratory compromised situations requiring CPAP/Bipap, high flow oxygenation as well as multisystem compromised patients. We are trained in caring for complex IV drips requiring telemetry monitoring as well as general medicine cases such as renal failure and acute on chronic illnesses.

 

5 Main is a 27-bed medical telemetry unit which provides care to patients with a variety of medical diagnoses. 

 

5 South is a 22-bed medical telemetry unit managing a patient population of general medical diagnoses, with specialty in peritoneal dialysis.

 

6 Main is a 34-bed general surgical unit.  We provide care to patients who are recovering from GI/GU surgeries, urological procedures, breast surgery, thyroid surgeries and patients with medical health issues. 

    

7 South is a 32-bed oncology unit specializing in the care of the oncology patient.

 

Tower 3 is a 55-bed ortho/neuro surgical floor managing the care for a varied patient population ranging from neurological issues (strokes, MS exacerbation, seizures, long-term epilepsy monitoring) to neurosurgical (backs, necks, brains), to trauma (falls, motor vehicle accidents, and victims of violent crimes). 

 

Job Description:

Under the general supervision of the Nurse Manager, provides professional nursing care at the beginner practitioner level within an assigned unit according to established procedures, protocols and regulations. Coordinates care planning with other disciplines. May require specialized skills depending on unit assignment.

 

Duties and Responsibilities

  1. Provides and coordinates direct care in accordance with the principles of Relationship Based Care to patients based on the nursing process and according to the clinical standards of the hospital. Acts as a primary nurse for patients on the unit. Synthesizes patient data to develop outcome-driven plans.
  2. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care.
  3. Appropriately organizes and prioritizes time based on patient condition and expected workload. Identifies changes in patients’ status and acts accordingly. Knows and can activate resources to achieve outcomes for patients.
  4. Seeks mentoring and coaching from the CN I, II & III as needed.
  5. Identifies and communicates individualized patient needs for inter-shift report and transfer/discharge. Includes patient/family in planning. Collaborates with all disciplines in evaluation of patient progress.
  6. Appropriately and correctly documents the patient response to interventions and progress toward outcomes. Identifies and documents variances from pathway or plan of care. Appropriately utilizes flow sheets, progress notes, and/or electronic medical record for documentation. Validates compliance with system and unit-specific clinical initiatives, i.e. pain scores, restraints, fall precautions, etc.
  7. Keeps practice current by participating in performance improvement efforts. Complies with changes in clinical standards and policies. Participate in data collection for performance improvement activity. Recognizes the value of evidence based research as it applies to best practice.
  8. Seeks learning opportunities and resources beyond own work environment and incorporates into practice the new knowledge and skills acquired.
  9. Promotes an environment conducive to professional practice, interdisciplinary collaboration, and patient and staff safety. Supports Shared Decision Making by unit committee membership or by supporting and helping fellow co-workers attend committee meetings and/or work on SDM projects. Participates in staff meetings and unit activities. Uses opportunities in the clinical setting to teach and support others, one-on-one or in groups (e.g. Students, other hospital personnel).
  10. Begins to provide leadership for problem solving and goal formation. Negotiates for a win-win solution.
  11. Delegates aspects of care to other nursing personnel in order to meet patient needs and to maximize team performance. Helps assistive personnel organize and set priorities for work. Monitors completion of tasks that are assigned or delegated to the patient care team.
  12. Promotes collaboration and cooperation among physicians, nursing staff, and other members of the interdisciplinary team. Contributes to effective communications among all team members. Begins to display conflict resolution and negotiation skills to maximize team performance.
 

 

 

Qualifications

  • Minimum Experience: Graduate-eligible of an accredited nursing program
  • Minimum Education:  Associate's Degree
  • Preferred Education:  Bachelors of Science, Nursing (BSN)
  • Required Certification:  PA Registered Nurse license or Temporary Graduate Registered Nurse license required in order to begin employment; BLS Certification;
  • Physical Demands:
    • Prolonged, extensive, or considerable standing/walking
    • Lifts, positions, pushes and/or transfers patients
    • Considerable reaching, stooping, bending, kneeling, crouching
