Job Description:

Under general supervision, and in collaboration with other clinical leaders, provides clinical direction and management of activities related to the care management of assigned patient population, including clinical policy development and implementation, clinical data management and reporting, achieving clinical outcomes, improving cost-effectiveness, and interdisciplinary collaboration and coordination in the management of patient care.

Duties and Responsibilities

Manages the care management program for assigned patient population including clinical policy development and implementation, clinical data management and reporting, achieving improved clinical outcomes, improving cost-effectiveness, and interdisciplinary collaboration and coordination in the management of patient care.

Collaborates with other clinical leaders to advance clinical process, system improvement, and sharing of resources across the system.

Develops and implements goals for operational area which are synergistic with the Blue Book objectives. Consistently monitors progress toward goals. Supports achievement of overall Nursing department goals.

Directs a collaborative planning process with clinicians within the service line to meet the needs of the assigned patient population.

Manages cost-effective clinical staffing across the continuum of care.

Collaborates with, and mentors the management team within the service line.

Coordinates clinical performance improvement activities within the clinical area.

Supports and mentors shared decision-making process and clinical advancement philosophy.

Utilizes lean principles to foster operational improvement in patient centeredness, quality of care and cost-effectiveness.

Maintains knowledge of regulatory requirements and seeks to keep clinical areas continuously in compliance.

Common Expectations:

Recommends staffing levels, selects and assigns staff, evaluates performance, evaluates/tests competencies (as applicable), provides orientation, training and continuing education of staff, and initiates or makes recommendations for personnel actions.

Develops budget(s) for the department/unit/entity and allocates funds within budget(s) to accomplish objectives. Monitors variance against budget(s).

Coordinates and integrates services within the department/service line/entity, with other departments/service lines/entities, and with the System's primary functions. Participates in the selection of outside services (if needed).

Establishes and implements unit goals and objectives in keeping with nursing administration goals and objectives. Acts as liaison between nursing administration and medical staff.

Continuously assesses and improves the department/service line/entity/System's performance. Maintains appropriate quality control/assessment programs (if applicable).

Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).

Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analysis as required.

Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.

Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Qualifications

  • Minimum Experience: 3 - 5 years leadership experience in surgical services 
  • Minimum Education: Bachelor's Degree
  • Required Certification: Registration and current RN license in PA required ; CNOR, CPAN, or CAPA certification required.
