General Summary:

 

Under the general supervision of the Medical Director, Continuing Care Services, WellSpan Health, performs a wide variety of administrative, operational and fiscal functions including, but not limited to, planning, directing, budgeting, coordinating the administrative operations and integration activities of Continuing Care services across the system.  Works collaboratively with WellSpan Medical Group, Services Lines, Acute Care Entities, Case Management, Home Health Care, Population Health, and others to execute tactics that advance system’s 2025 strategic plan.

 

Duties and Responsibilities:

 

 

Under direct supervision of the Medical Director, Continuing care services, provides administrative leadership to various programs under purview of WellSpan Continuing Care services.

Leads development, optimization, and growth of WellSpan at Home (Connected Care at Home, Hospital at Home, Advance Illness Management, Primary Care at Home) services systemwide.

Provides leadership and support to team that manages WellSpan’s network of post-acute preferred partners to optimize network performance to deliver high quality clinical care leading to improvement in outcome measures such as skilled nursing facility length of stay, readmission rate and others, to achieve a reduction in total cost of care.

Provides administrative leadership to develop, grow and optimize cross-continuum palliative care services across the system. Spearheads and oversees clinical services, program development, personnel, scheduling, budget, productivity, and performance improvement.

Provides leadership to Horizon/Advance Care Planning team responsible for implementing and maintaining system-wide, comprehensive Horizon Planning programs for WellSpan Health.

Monitors program activities and, where appropriate, physician participation, recording, reporting, and analyzing statistical data, including financial data, to determine levels of participation, adherence to national benchmarks and to identify and develop plans to address issues, as needed.

Participates in development of strategies to implement new services and emerging care models as appropriate.

Facilitates positive working relations with and between the medical staff and assigned areas.

Collaborates with clinical areas to ensure quality care.

Analyzes, records, and reports statistical data, including financial data.  Identifies pertinent data elements for measurement.

Seeks and maintains current knowledge related to patient regulatory and policy changes of importance to areas of oversight.  Ensures compliance with all applicable federal and state regulations.

Develops and fosters relationships with all appropriate regional post-acute leadership teams to ensure adherence to ongoing focus of improving care transitions, enhancing patient safety, improving performance related to identified quality metrics and identification of opportunities to improve clinical competency in these settings.

Develops and prepares annual budget.  Monitors expenditures.

Participates with leaders from the governing body, management, medical staff and clinical areas in decision-making structures and processes.

Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

 

Qualifications

 

 

3 - 5 years as a Senior Manager or Director 

Master's Degree

Healthcare or business administration

Experience in one or more levels of post-acute care

Excellent communication, interpersonal, managerial and computer skills
