Diabetes Quality Coordinator Supervisor
Full Time

Monday - Friday; 8:00am - 4:30pm
80 hours / 2 weeks

Job Description:

Under the general supervision of the Clinical Manager/Director, the supervisor & diabetes quality coordinator is responsible for overseeing the day-to-day operations of the diabetes self-management education and support (DSMES) service at all sites of the program, providing both clinical and administrative oversight of the program and staff. The Quality Coordinator ensures that the National Standards (NSDSMES) are met and maintained.

Duties and Responsibilities:

1. Provides oversight of the planning, implementation and evaluation of the DSMES service at all sites, including scheduling, staffing, marketing, implementing, and evaluating the program.

2. Responsible for maintaining ADA Recognition and participating in the evaluation of the DSMES service’s effectiveness including appropriate documentation and education requirements to ensure continual recognition in meeting the national standards for DSMES

3. Develop and maintain teaching curriculum for classes.

4. Arrange and coordinate the activities of the Diabetes Advisory Group. Liaises between the DSMES team members, the Advisory Group, other departments and administration.

5. Monitor and facilitate maintenance of DSMES team members qualification (CE credits, training, competency, licensures, and registrations).

6. Provide effective diabetes instruction (both group and individual), nutritional counseling and follow-up as needed to ensure continued maintenance of care for patients.

7. Collaboratively develop educational goals, learning objectives and a plan for educational content and teaching methods with DSMES program participants.

8. Provide appropriate and timely feedback to the referring physician.

9. Track and maintain inventory of supplies pertinent to the needs of the department.

10. Apply clinical skills in a variety of preventative health programs to the community

11. Organize and provide staff meetings, in-services, support groups and participate as a member of the Speakers Bureau of the hospital.

12. Represent department on interdepartmental committees related to special projects.

Qualifications:

  • Minimum Experience:
    • 2 to 3 years in conducting diabetes education
  • Minimum Education:
    • Bachelor’s Degree

    • Completion of an Accreditation Council for Education in Nutrition and Dietetics (ACEND) approved program

  • Required Certification:
    • Registered Dietician Nutritionist (RDN) Certification through the Commission on Dietetic Registration (CDR); Pennsylvania State Licensure of Dietician-Nutritionist (LDN); Current certification in BLS
  • Preferred Certification:
    • Certified Diabetes Educator
