Job Description:

Under the general supervision of the Director-Community Health Improvement, establishes trusting relationships with identified disparate population groups that strengthens the patient-provider relationship and improves patient care. In addition, disseminates health information in a culturally relevant manner and builds individual and community capacity to address health care needs.

 

Duties and Responsibilities

  1. Assesses needs of population, patient or individual on an ongoing basis. Identifies barriers to care and makes appropriate recommendations for follow-up or referral.
  2. Educates community members, either individually or in groups, about how to access and/or use the health care system. Shares relevant concerns from community members with medical providers or others within the health care system.
  3. Identifies and utilizes culturally appropriate outreach strategies to provide specific health promotion and disease prevention education to specific population groups. This may include, but is not limited to: developing and coordinating a network of community lay leaders; implementing community education and screening events, and; conducting relevant follow-up with participants.
  4. Assists, as requested, in the coordination of care for clients/patients. This may include, but is not limited to: facilitating enrollment in public health insurance and charity care; conducting site visits with at-risk patients/clients, and; providing a cultural context for effective medical care and behavior change as part of a clinical care team.
  5. Evaluates program outcomes/service effectiveness through tracking participation levels, conducting participant surveys, reviewing chart records, and engaging patient in regular follow-ups.
  6. Documents and maintains accurate and confidential records of client interventions.
  7. Actively participates on community coalitions and boards as an advocate for specific populations and as a liaison for WellSpan Health

 

Qualifications:

 

  • Minimum Experience:
    • One year in a community related position

 

  • Minimum Education:
    • High School or G.E.D.

 

  • Required Courses:
    • Basic knowledge of disease prevention/health promotion

 

  • Skills:
    • Good communications and interpersonal skills; ability to make meaningful observations and
    • reports; proven organizational and motivational skills; ability to work cooperatively as part of a
    • team and with minimal supervision; ability to communicate in Spanish preferred
    • Must possess a valid PA driver's license; Residency in service area preferred
