Job Description:
The Clinical Transformation Consultant role will combine strategic thinking, change management methodologies and deep industry knowledge to lead and facilitate improvement initiatives across the care continuum. This member of the population health team will be aligned with a portfolio of clinical service lines and will embed with each to enable total cost of care transformation. The role will partner closely with clinical and operational leaders to ensure that improvement projects are focused, aligned with population health strategies, and metrics are in place to measure impact. Must be flexible to fill a variety of roles, most importantly the ability to coordinate highly complex projects requiring the efficient assembly of diverse stakeholders and corporate functions. This role will partner closely with a Clinical Transformation Business Analyst, who will be responsible for data development, business modeling and providing analytic insights to support transformation efforts. This position will be expected to promote a cycle of continuous improvement in pursuit of WellSpan’s goal to become a highly reliable, and affordable healthcare provider. Position reports to the Director of Clinical Transformation.
Duties and Responsibilities
- Leads and consults a portfolio of “total cost of care” initiatives across assigned service lines and clinical departments.
- Builds effective relationships with clinical and operational leaders, supporting a continuous cycle of opportunity identification, data driven prioritization, project scoping, design of countermeasures and project execution.
- Understands the drivers of success under value-based care models, and effectively translates these to communicate with, and coach a diverse audience, inclusive of front-line professionals.
- Leverages a working knowledge of finance, electronic health records and data systems to proactively support projects and facilitate their efficient and effective collaboration with corporate departments.
- Coordinates project needs across the enterprise, drawing on the specific expertise of finance, performance improvement, and EPIC informatics as needed.
- Collaborates closely and directly with a business analyst partner, to seek and develop insights from available data systems. Collaboratively sets targets and tracks incremental progress towards goals.
- Partnering with the performance improvement as needed.
- Monitors savings and improvements from projects, ensuring savings calculations are validated and savings resulting from project successes are properly allocated.
- Regularly develops and reports update summaries for reports to service lines and system population health leadership.
- Proactively reviews industry trends and benchmarks. Internal benchmark variance and industry trend differences are studied to ensure strategies and project executions have been and remain successful.
- Continuously monitors project outcomes such as ED and hospital utilization rates, length of stay, readmissions, and cost per case. Makes recommendations and adjustments when projects and programs are missing anticipated improvement targets.
- Supports a culture of continuous shared learning and brings a “fail forward” and “fail fast” perspective to WellSpan’s total cost of care transformation journey.
Qualifications
- Notes:
- Other combinations of formal education, training and experience may be considered.
- Minimum Experience:
- 3- years building rapport with clinical leaders and leveraging data to drive change
- Clinical background (LPN, RN, LSW etc.) or experience with clinical operations
- Pref Field of Expertise: Healthcare Management, Change Management, Population Health
- Minimum Education:
- Bachelor’s degree
- Preferred Education:
- Master’s Degree in related field
- Skills:
- Intermediate computer skills; project management skills; excellent communication, problem solving and analytical skills.
