General Summary:


Seeking a clinical supervisor responsible for delivery and evaluation of patient care services to patients on assigned team(s). Assists in implementing new Home Health programs as well as evaluating and updating current programs and policies. Assumes the responsibilities of the Manager as designated.




Duties and Responsibilities:


1 Ensures that quality and efficient care is provided to all patients.
2 Monitors productivity of respective staff and provides additional guidance to staff that are not able to meet the established productive requirement.
3 Reviews appropriate documentation for content, accuracy, and completeness. Also ensures that all documentation is completed within the time frame required.
4 Identifies un-met patient needs and reports findings to appropriate Branch Manager by preparing a statistical data analysis verifying the need.
5 Incorporates the findings of the clinical quality review process into team meetings and individual case conferences to ensure proper utilization of services to respective patients.
6 Makes home visits as required/needed (Expectation 25-50% time spent with staff in field or training environment).
7 Ensures all information is communicated accurately and consistently.
8 Provides clinical assistance to respective scheduler as requested. Informs scheduling department of any clinical patient/staff related issues in the event patient care is/could be impacted.
9 Demonstrates leadership by example while promoting Just Culture and positive interactions throughout the organization.


B Related to Staff
10 Completes/assists in performance appraisals for staff using available performance data within the VNA policies.
11 Establishes/assists in development plans for all respective staff. Ensures clinical competencies of all field staff members. Assumes ultimate responsibility for orientation of new field staff.
12 Completes home visits to mentor, train and assess individual clinicians job performance and skill competency and to evaluate patient satisfaction, workflows and compliance a minimum of 25% of hours worked.
13 Promotes open and effective communication throughout the team, organization and System.
14 Provides positive leadership to all staff and demonstrates support of the organization.
15 Provides after-hours and on-call staff support as well as visits and consultation on a scheduled, rotating basis.


C Related to Home Health Management/Hospice
16 Participates in the planning, implementation, management and evaluation of the Home Health & Hospice Services Program as requested.
17 Participates in the identification and development of new clinical policies and the regular review and updating of existing clinical policies as indicated.
18 Coordinates services with other community agencies as directed.
19 Participates on special committees as requested.


Required Skills

Minimum Education              

Bachelor’s Degree in nursing or rehabilitation therapy




Minimum Experience           

3 - 5 years clinical nursing or therapy experience (an emphasis in home care is preferred)



Required Certification/Registration          

Registration and current RN license in PA; or appropriate Rehabilitation Therapy license (i.e., Physical Therapy, Occupational Therapy, Speech Pathology); Current CPR for Healthcare Providers Certification



Demonstrated knowledge in clinical skills;

Ability to exercise leadership as well as to work in a collaborative team relationship with professionals;

Ability to accept and utilize leadership direction and consultation; A

bility to communicate effectively orally and in writing;

Possesses good conceptual skills.

Ability to use the problem solving approach in assessing, analyzing and planning;

Ability and desire to develop professionally and to mentor others.  

Ability to use lean process improvement to implement and evaluate system changes;


Must possess a valid driver’s license in the current state of residence.  Driving record must meet requirements established by WellSpan Risk Management.
