General Summary:


Under the general supervision of the Pharmacy Coordinator, provides consultative services to medical, nursing and pharmacy staff.  Documents clinical activities and outcomes.  Compounds medications and provides prescribed medications according to professional standards and practices.


Duties and Responsibilities:


  • Conducts direct patient care activities utilizing a consistent approach that reflects the philosophy of pharmaceutical care. Reviews all nonemergent orders prior to the first dose administered.  Monitors patient medication profiles daily.  Reconciles medications during all changes in level of care, including transfers and postoperatively.  Develops individualized treatment and monitoring plans for patients.  Performs medication histories for readmitted patients.  Reconciles medications upon admission for patients admitted as inpatients.  Reconciles medications for targeted patients prior to discharge to ensure continuity of care.
  • Provides medication-use education to patients and caregivers. Educates targeted patients about medications.  Counsels patients who are prescribed high-risk medications or complex medication regimens on their discharge medications during inpatient hospitalization.
  • Manages medication therapy in collaboration with other members of the health care team. Provides initial pharmacokinetic evaluation for narrow therapeutic index medications.  Oversees compounding of orders for parenteral nutrition solutions.  Provides drug information to the health care team.  Provides programs for patients that prevent disease and improve health.  Participates in anticoagulation management, nutrition support services, antimicrobial stewardship and on resuscitation teams.  Streamlines medication orders (e.g., i.v.-to-oral medication conversions).  Provides pharmacokinetic evaluation, monitoring and dosing, and renal dosing adjustments for medications that are renally excreted. 
  • Provides Pharmacy management and/or oversight of: a) medications in all care locations; b) all investigational medications; c) preparation of injectable chemotherapy; d) medication waste, returns and recalls; e) the process of reconciling controlled-substance waste; f) compounding of formulations; and g) automated dispensing cabinets.  Standardizes medication doses to improve safety and efficacy.  Ensures appropriate medication label components and label quality, and safe medication storage.
  • Provides medication event prevention, monitoring and reporting. Provides education to allied health professionals on medication safety issues.
  • Ensures that technology provides patient safety. Uses technology and information systems to ensure the proper distribution and use of medications.  Participates in the assessment of technology.
  • Participates in quality, safety and performance initiatives related to medication use at organization and department levels. Applies infection-control principles to medication dispensing and administration.
  • Participates in the education and/or training of doctor of pharmacy students, pharmacy technicians and pharmacy residents. Collaborates in clinical or operational system/process research activities.
Required Skills


  • Pharmacy Bachelor's Degree or Pharm D 
  • Completion of 5 year program from accredited school of pharmacy
  • Current license to practice Pharmacy in the Commonwealth of PA 
  • PGY1 (preferred)
  • Previous hospital pharmacy experience (preferred)
  • Excellent interpersonal and communication skills. Must effectively communicate with all levels of customers (physicians, staff, patients, etc.).
  • Must be a team player and able to work independently with daily responsibilities
