Clinical Improvement Quality Specialist

Job Description:

Under general supervision, performs a variety of duties including, but not limited to, planning, coordination, consultation, design and collaboration related to the reporting and/or abstraction, quality assessment and analysis of data from sources such as health care information databases and systems including, but not limited to, the electronic health record (EHR) Quantros, GWTG Registries, PHC4 Web, , Premier, Metrics Dashboard and publicly reported data and related websites. Develops queries of computer databases to measure performance. Works collaboratively with the Hospital leadership, Service Lines, and regional colleagues in continuous quality improvement for excellent patient care. Validates all reportable data results through qualitative and quantitative analyses to confirm findings accuracy. Presents data in appropriate formats. Prepares and organizes information for meetings and audit visits and assists in coordination of deliverables. Meets all regulatory required data entry and filing deadlines. Participates in development and implementation of quality assessment indicators.

 

Duties and Responsibilities

 

  1. Provides timely, high quality data reporting and abstraction and related clinical information collection.
  2. Coordinates clinical data and audit finding needs to improve patient care delivery, identifies opportunities for improvement and achieves desired patient outcomes.
  3. Performs structured audits of medical records. Reviews records for completeness, accuracy, legibility, follow up of abnormal test results and other indicators.
  4. Compiles data for and prepares regularly scheduled or special reports.
  5. Explains data results as necessary.
  6. Evaluates procedures, resolves problems and implements changes to improve production.
  7. Verifies the data integrity for statistical analysis.
  8. Establishes data bases as needed by leadership.
  9. Provides consultation and recommendations based on analytical results to improve business processes or outcomes with the provision of on-going education and support.
  10. Formulates validation inter-rater reliability and quality assurance strategies and methods to ensure accurate and reliable data, ensuring appropriate departmental/service line/project and corporate standards are met and the data is correct (may include report generation and comparison). Proactively corrects errors as identified.
  11. Works with customer to define informatics and reporting requirements, including System leadership.
  12. Collaborates with appropriate Service Lines/IS team for any identified problems and/or outcomes for further investigation or recommended action.
  13. Participates as a professional team member in professional committee and survey discussions by attendance and active, thoughtful participation.
 

Qualifications

 

  • Minimum Experience:
    • 3 years

 

  • Minimum Education:
    • Bachelor’s Degree

 

  • Minimum Field Of Expertise:
    • Relatable experience in abstraction, data reporting, quality assurance and other forms of clinical information collection; including, but not limited to, clinical documentation improvement, regulatory, care management, healthcare auditing, etc..

 

  • Preferred Certification:
    • Registered Health Information Administrator (RHIA); Certified Health Data Analyst (CHDA); Certified Professional Healthcare Quality (CPHQ); licensure as a medical professional or registration and current RN license in PA

 

  • Skills:
    • Medical record review and reporting, abstracting & coding; database & advanced Microsoft Office and other computer skills absolutely required; excellent professional written and verbal communication skills, Excellent teamwork skills; knowledge of appropriate tools & methodologies, knowledge of medical terminology, ability to work independently and as an integral part of a collaborative team, knowledgeable in the application of performance improvement models, able to conceptualize and convey the importance/impact of data integrity in terms of patient management, study results, cost efficiency, quality of service, and the goals of clinical improvement and related project management., PDSA knowledge
