Position Summary:

 

General Summary:

Under general supervision, performs a variety of duties including, but not limited to, planning, coordination, consultation, design and collaboration related to the abstraction, quality assessment and analysis of data from sources such as health care information databases and systems including, but not limited to, Quantros, GWTG Registries, Open Heart Database, Cardiac Care, PHC4 Web, Atlas, SSL, Business Objects, claims, utilization data and clinical records such as Power Chart and clinical patient listings. Develops queries of computer databases to measure performance. Works collaboratively with the Service Lines, Clinical Effectiveness Teams and Hospitals in continuous quality improvement for excellent patient care. Validates all reportable data results through qualitative and quantitative analyses to confirm findings accuracy. Presents data in appropriate formats. Meets all regulatory required data entry and filing deadlines. Participates in development and implementation of quality assessment indicators.

Duties and Responsibilities:

 

   1  Provides timely, high quality data abstraction and related clinical information collection.

   2  Enters submission of registry specific requirements into various AHRQ clinical registries for patient outcomes.

   3  Coordinates clinical data needs to improve patient care delivery, identifies opportunities for improvement and achieves desired patient outcomes.

   4  Performs structured audits of medical records. Reviews records for completeness, accuracy, legibility, follow up of abnormal test results and other indicators.

   5  Compiles data for and prepares regularly scheduled or special reports.

   6  Analyzes data for significant trends or occurrences. Explains data results as necessary.

   7  Evaluates procedures, resolves problems and implements changes to improve production.

   8  Verifies the data integrity for statistical analysis.

   9  Establishes data bases as needed by the Clinical Improvement Team.

  10  Provides consultation and recommendations based on analytical results to improve business processes or outcomes with the provision of on-going education and support for the Clinical Improvement department and for Quantros users.

  11  Formulates validation quality analysis strategies and methods to ensure accurate and reliable data, ensuring appropriate departmental/service line/project and corporate standards are met and the data is correct (may include report generation and comparison). Proactively corrects errors as identified.

  12  Works with customer to define informatics and reporting requirements, including System leadership.

  13  Collaborates with appropriate Service Lines/IS team for any identified problems and/or outcomes for further investigation or recommended action.

  14  Participates as a professional team member in clinical data discussions by attendance and active, thoughtful participation in Clinical Effectiveness Teams, Meaningful Use, Utilization and/or other practice specific meetings or discussions for the purpose of clinical improvement.

 

Required Skills

Bachelor’s Degree

Minimum Experience 3 - 5 years

Healthcare quality assurance; experience in a managed care setting desirable

Registered Health Information Administrator (RHIA); Certified Health Data Analyst (CHDA); Certified Professional Healthcare Quality (CPHQ); licensed as a medical professional preferred

Registration and current RN license in PA preferred
