Job Description:

Under the general supervision of the Director-Spiritual Care & Education, provides spiritual counseling and guidance to patients, families and staff and performs or arranges for religious services to meet patient needs.

 

Duties and Responsibilities

 

  1. Participates in a spiritual assessment of the patient/family/caregivers, as indicated.
  2. Facilitates the development of a plan of care to meet the identified spiritual needs in conjunction with the interdisciplinary team.
  3. Provides direct spiritual care to patients/families/caregivers/staff, when needed.
  4. Serves as liaison to community chaplains and spiritual counselors, as indicated.
  5. Documents all patient/family/caregiver contacts and provides additional written, pertinent information as indicated.
  6. Participates in patient care conference and interdisciplinary team meetings.
  7. Provides consultation, education and support to the interdisciplinary team on spiritual care.
  8. Provides for funeral or memorial services or other services as requested by patient/families.
  9. Provides bereavement support.
  10. Provides spiritual support to staff..
  11. Meets regularly for coordination of working together.

 

 

Qualifications

  • Minimum Experience:
    • 2 - 3 years

 

  • Minimum Education:
    • Bachelor's Degree

 

  • Preferred Field Of Expertise:
    • Hospice/home health

 

  • Required Certification:
    • Ordained/Endorsed by recognized religious body/proper ecclesiastical authority

 

  • Preferred Certification:
    • Certified (or eligible) Chaplain by Association of Professional Chaplains (APC)

 

  • Required Courses:
    • Graduate of accredited seminary or school of theology

 

  • Skills:
    • Ability to work as member of interdisciplinary team; Comfort in an interfaith setting; Ability to accept different lifestyles, cultures, beliefs and values; Ability to network with community clergy and congregations; Knowledge of and commitment to Hospice philosophy of care; Must possess a valid driver’s license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.

 

  • Note: This position requires the person to visit clients and their families in their homes. It also provides staff care for the Hospice team . It frequently prepares and conducts funeral services for clients.
