Position Summary:

 

General Summary:
Under the general supervision of the Director-Spiritual Care & Education, provides spiritual counseling and guidance to patients, families and staff and performs or arranges for religious services to meet patient needs.

Duties and Responsibilities:

1 Participates in a spiritual assessment of the patient/family/caregivers, as indicated.
2 Facilitates the development of a plan of care to meet the identified spiritual needs in conjunction with the interdisciplinary team.
3 Provides direct spiritual care to patients/families/caregivers/staff, when needed.
4 Serves as liaison to community chaplains and spiritual counselors, as indicated.
5 Documents all patient/family/caregiver contacts and provides additional written, pertinent information as indicated.
6 Participates in patient care conference and interdisciplinary team meetings.
7 Provides consultation, education and support to the interdisciplinary team on spiritual care.
8 Provides for funeral or memorial services or other services as requested by patient/families.
9 Provides bereavement support.
10 Provides spiritual support to staff.

 

Required Skills

Must live within a 20 minute response time

Minimum Education: Bachelor's Degree

 

Required Experience

Minimum Experience    2 - 3 years

Req Cert/ Registration: Ordained/Endorsed by recognized religious body/Proper Ecclesiastical authority
