Certified Peer Specialist Lead - York Community Peer Support - Day/Eve

General Summary

Contributes in a peer support capacity to facilitate recovery and enhance wellness. Serves as a role model for recovery for both staff and peers, and partners with peers in developing recovery tools and building support networks. Has two primary functions: direct peer support (with 65% productivity for billable services) and supervisory activities for monitoring certified peer specialist's work and outcomes. (3-5 FT equivalents).

Duties and Responsibilities

Essential Functions:

  • Trains and orients new staff to duties and responsibilities of a Certified Peer Support position.
  • Monitors and maintains schedules of assigned CPSs.
  • Monitors CPS documentation in order to meet program requirements.
  • Provides additional support to CPS staff in the community when needed.
  • Monitors productivity of CPSs assigned.
  • Provides marketing and trainings within the community as needed.
  • Meets weekly for face to face supervision with all assigned staff. Documentation of these visits must be given to the program Director.
  • Meets weekly with program Director for supervision.
  • Participates in performance reviews and performance issues.
  • Assists designated peers with rehabilitative social support, advocacy, educational and vocational mentoring, and self-maintenance.
  • Provides information on accessing community resources.
  • Assists in transportation for peers to necessary appointments, support and outreach.
  • Provides reminder calls to designated peers to offer reminders and verbal supports in relation to their recovery.
  • At the request of the peer, assists family members in obtaining support resources, as well as educational information regarding recovery tools.
  • Promotes participation in WRAP and Psychiatric Advance Directives (PAD) planning among peers.
  • Helps individual peers complete their personal WRAP plans and PADs.
  • Models coping techniques and self help strategies.
  • Encourages and provides support for peers to advocate for themselves.
  • Reinforces hope and the potential for recovery to peers and others.
  • Partners with peers to create and attain goals on the Individual Recovery Plan.
  • Completes necessary documentation for each peer encounter (i.e. service activity, contact sheets, etc) within 48 hours of encounter.
  • Completes all necessary paperwork during the intake, and updates paperwork as needed.
  • Completes monthly log of services provided.
  • Attends all required trainings, in-services, staff meetings, etc.
  • Meets with supervisor for individual supervision on a weekly basis.
  • Meets with team on a bi-weekly basis for staff meeting and peer reviews.
  • Assists Peers with creating an Individual Recovery Plan. Initial IRP must be completed within 30 days of first face to face visit. A six month review from the date of the most current IRP will be required for continuation of services.
  • Reviews Individual Recovery Plan with peers every 6 month or earlier as needed.
  • Acts as a liaison with other community agencies/groups, funding sources, and businesses that relate to the services provided to peers.
  • Acquires knowledge of resources in assigned county or counties.
  • Provides onsite visits to other community agencies in order to educate them about the peer services available.
  • Communicates on an as needed basis with providers regarding issues relevant to continuity of care.
  • Monitors wear and tear on furnishings and equipment. Reports concerns and needs to supervisor.
  • Provides after hours telephone support to enrolled peers as assigned. (Pager rotated among staff on a monthly basis.)

  • All other duties as assigned by supervisor.
  • Maintains an excellent working relationship with all providers in each county served.

Qualifications

Minimum Education:

  • High School Diploma or GED Required
  • Associates Degree Of Arts degree as Medical Assistant or related health field. Preferred

Work Experience:

  • 1 year Supervisory experience. Required and
  • A self-identified mental health consumer who has received or is receiving state priority group services as defined in MH Bulletin OMH-94-04, Serious Mental Illness: Adult Priority Group. Required and
  • 1 year Of paid or volunteer experience within a three year period. Required and
  • 1 year Experience in navigating the mental health system (i.e. crisis services, vocational rehabilitation, recreational activities, and human services, etc.). Required and
  • A history of coping with own mental health issues and maintenance of own recovery. Required and
  • Medical Assistance or related health field experience. Required

Licenses:

  • Certified Peer Specialist Upon Hire Required and
  • Certified Peer Support Supervisor Upon Hire Required

Driver's License Statement:

Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.

Knowledge, Skills, and Abilities:

  • Excellent human relations and oral/written communication skills.
  • Basic computer skills including email, Word, and internet.
  • Knowledge of and ability in the care and support of individuals in a specific age group.
