Job Description:

Under the general supervision of the Director-Quality and Clinical Improvement, reviews, administers, coordinates and supports the System-wide 24/7 Central Alert system. Using advanced clinical assessment skills and knowledge with human response to life threatening problems, receives and screens care alerts by real time review of the patient electronic health record for early identification of protocol driven interventions. Utilizing nursing diligence, communicates for action as appropriate, related to alert and record identified order set and care protocol interventions. Working remotely and collaboratively by telemonitoring technology, acts as a real time clinical support for the HELP team, bedside nursing and/or physician/Advanced Practice Clinician on patient conditions that require immediate attention. Is responsible for on-site demonstration as needed, training and training material development and quality assurance review, scoring and communication and assumes supervisory responsibilities for the Central Alert Team and related workflow, under the direction of the Director.

 

Duties and Responisbilities

Essential Accountabilities:

  1. Performs diligent review of electronic health record systems data and protocol driven alerts for screening of necessary and appropriate interventions, using clinical assessment and knowledge skill and pre-determined/defined order set(s).
  2. Provides real time identification and communication of missed opportunity or care gaps related to alert.
  3. Provides timely and appropriate communication of identified and related assessment to appropriate clinical staff and/or HELP team.
  4. Prepares professional communication verbal/written of identified findings and interventions, including thorough and complete nursing documentation.
  5. Maintains a thorough knowledge and understanding of evidence based/best practice.
  6. Utilizes strong decision making, systems thinking and computer literacy skills in a remote practice environment, to assist in support of coordinated and timely patient care intervention.
  7. Performs virtual “rounding” regarding patient assignment and alerts, reporting as directed.
  8. Follows escalation process/procedure to address real-time care concerns.
  9. Revises and monitors educational materials to ensure that they are coordinated, understandable and meet the program standards.
  10. Protects patient confidentiality by promoting professional staff communications.
  11. In downtime, follows a set procedure for several other job responsibilities including; review and decision making of incidental findings and the incidental findings registry as a care coordinator, and EWS alerts of various types.
  12. Performs Central Alert Team quality assurance through interrater reliability scoring and communication
  13. On Call as needed, in Management support.
 

NonEssential Accountabilities:

  1. Responsible for the care and maintenance of department/entity equipment and supplies.
  2. Attends meetings as required, and participates on committees as requested.
  3. Maintains ongoing education for licensure, certifications and as required within work specialization.
  4. Performs other related duties as identified.
 

Common Expectations:

  1. Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population.
  2. Senior Central Alert Team Nurse
  3. Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards. Coordinates all department policies, and documents. Maintains training binder, sharepoint site and departmental files.
  4. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
  5. Average quality scores (interrater reliability) 90% or better.
  6. Actively coaches; utilizing professional verbal and written communication to coach staff in a positive, helpful and productive way.
  7. Work with the Central Alert Team Management to conduct special projects or reviews as requested.
 

Qualifications

  • Notes:
    • Graduate of an accredited nursing program.
  • Minimum Experience:
    • Greater than 3 years
    • Min Field of ExpertiseNursing experience in Emergency Department, Critical Care, related Management or sepsis specific setting/role; Proven experience as a Central Alert Team Nurse and extensive education/training in sepsis
    • Req Cert/RegistrationRegistration and current RN license in PA; Basic Life Support (BLS) certification required
    • Pref Cert/RegistrationCertified Critical Care Registered Nurse (CCRN) or Certified Emergency Nurse (CEN); or other Professional Certification
  • Minimum Education:
    • BSN or equivalent experience
  • Physical Demands:
    • Mostly sedentary work
    • Occasional prolonged sitting/standing
    • Occasional reaching, stooping, bending, kneeling, crouching
  • Skills:
    • OtherExcellent computer skills; Effective decision making. Highly organized. Proven outstanding interpersonal skills; demonstrated ability to function as a team player. Leadership skills. Knowledge of the Healthcare System. Excellent professional written and verbal communication skills, Excellent teamwork skills; knowledge of appropriate order sets and clinical protocols. Ability to work independently and as an integral part of a collaborative team
