Position Summary:

General Summary:

Under the general supervision, provides case management services to customers and maintains relationships with referral sources throughout the community.  Coordinates scheduling, treatment, communication and reporting with injured workers and local employers on behalf of Orthopedic Services.

Duties and Responsibilities:

The following are essential job accountabilities:

   1  Works with providers to ensure efficient and effective scheduling, treatment, discharge and/or follow up of injured workers.

   2  Oversees case management processes, including development of both paper based and electronic forms, policies and procedures in cooperation with the Program Director(s) or Office Manager(s).

   3  Develops and maintains relationships with referral sources throughout the community. 

   4  Coordinates scheduling of appointments, communication, and documentation in the electronic medical record and processing of forms for injured workers at the various offices. 

   5  Develops and presents educational programs and training on prevention of work-related injuries.

   6  Monitors the satisfaction level of patients and referral sources.

Required Experience
  • Minimum Experience              1 year or more
