Job Description:

Under direct supervision, performs a variety of duties related to facilitating the discharge planning process. Assists the Care Management team with the clerical functions. All tasks listed below may be expected in assigned daily work.


Duties and Responsibilities

Essential Accountabilities:

  1. Assists with clerical functions (e.g., filing, copying, scanning, emailing, telephone, taking minutes)
  2. Determine patient health insurance benefit information as requested by CMRN/CMSW.
  3. Reviews payor logs and enters authorization information into EMR/patient record.
  4. Receives denial information and records in EMR/patient record; forwarding same to assigned CMRN.
  5. Initiates SNF authorizations as requested by CMSW/CMRN; followed by documentation of payor determination in EMR/patient record.
  6. Completes discharge notification to varied insurance payors.
  7. Assists CM team with IMM notices to patient.
  8. Assist with medication pricing as requested by CMRN/CMSW.
  9. Coordinates after care issues with the appropriate team members.
  10. Supports the CM team by coordinating follow up PCP appointments.
  11. Compiles data for and prepares regularly scheduled or special reports, analysis, and statements.
  12. Reviews daily patient discharge list, routes incomplete VNA orders, identifies incomplete auth information, reviews final disposition Epic navigator for accuracy and completeness.
  13. Works and maintains Incomplete Auth work queue; obtains delinquent authorizations and records in EMR.


  • Notes:
    • Other combinations of formal education, training and experience may be considered.
    • BLS certification is required for this position.
  • Minimum Experience:
    • 2 – 3 years
  • Minimum Education:
    • High School or G.E.D.
  • Physical Demands:
    • Prolonged, extensive, or considerable standing/walking
  • Skills:
    • Basic computer skills; telephone etiquette; good communication skills; medical terminology preferred
    • all responsibilities, duties and skills required of personnel so classified.
