General Summary:

Under general supervision, provides pharmaceutical support services to patients, providers and other members of the Care Coordination Team (CCT) or Medical Neighborhood.

Duties and Responsibilities:

1 Participates in daily huddles, where appropriate, and makes recommendations in regards to medication therapy, dosing, costs or other pertinent clinical issues.
2 Accepts referrals/consults from the CCT or Medical Neighborhood for medication therapy management services.
3 Provides medication reconciliation for complex patient regimens.
4 Provides patient education to high risk patients to promote adherence to medication care plan.
5 Evaluates current drug use of managed patients seeking to identify and resolve potential and actual medication related problems including medication selection, improper dosing, adverse drug reactions, drug interactions, medications without indications and general polypharmacy.
6 Collaborates with inpatient pharmacy when managed patients are ready for discharge.
7 Provides patient drug therapy monitoring for the purpose of optimizing patient outcomes and minimizing adverse outcomes.
8 Provides pharmaceutical education to other health care professionals via presentations, formal utilization reviews, one-on-one interactions, or group huddles.

Required Skills

Pharmacy Bachelor's Degree required

Pharmacy Doctorate preferred

RPh and current PA licensure required; Pharm. D. preferred

Required Experience

1 - 2 years experience in the pharmacy field required
