***PART TIME - 24 HOURS BI-WEEKLY***

 

 

General Summary:

 


Under general supervision, coordinates all aspects of treatment, evaluation, support services and education for cancer
patients. Coordinates clinical documentation from referring and treating physicians for each patient.

 


Duties and Responsibilities:

 



1 Provides initial intake phone contact with cancer and/or symptomatic patients.
2 Introduces patient to the Health System.
3 Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment.
4 Provides individualized information, support, and education regarding cancer.
5 Coordinates appointments for consultations and physician visits.
6 Notifies primary care and specialty care providers when patient is admitted to the Hospital.
7 Follows patient via contact with inpatient nursing staff.
8 Participates in interdisciplinary nursing rounds for inpatients (discharge planning process).
9 Reinforces pre- and post-operative instructions and provides further education as appropriate.
10 Maintains two way communication with providers offices in order to improve the process of coordinating care for cancer patients.
11 Reports outcome of nursing/patient care/coordination activities to Oncology Service Line leadership.
12 Identifies patient and/or provider needs and develops quality programs to meet those needs. Ensures delivery of quality
services for patients.
13 Collaborates with outreach/education coordinators for all service lines in the preparation and delivery of community and
professional education programs.
14 Collaborates with research team in clinical trials enrollment and data collection.
15 Assists providers in teaching breast self-examination (BSE), as appropriate.
16 Serves as a resource to cancer support groups and other activities.
17 Revises and monitors educational materials to insure that they are coordinated, understandable, and meet the PFEAT
standards.
18 Protects patient confidentiality by promoting professional staff communications.


Required Skills

Minimum Education

Bachelor's Degree

 

 

Minimum Experience

3 - 5 years nursing experience

 

 

Req Cert/Registration

Registration and current RN license in PA
