Schedule & Location

Hours: Full Time, Day shift (80 hours every 2 weeks) 

Chambersburg Hospital with some outpatient work, as needed

Job Description

Under general supervision, coordinates all aspects of treatment, evaluation, support services and education for cardiology/heart failure patients. Coordinates clinical documentation from referring and treating physicians for each patient.

Duties and Responsibilities

  1. Provides initial intake phone contact with patients.
  2. Introduces patient to the Health System.
  3. Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment.
  4. Provides individualized information, support, and education.
  5. Coordinates appointments for consultations and physician visits.
  6. Notifies primary care and specialty care providers when patient is admitted to the Hospital.
  7. Follows patient via contact with inpatient nursing staff.
  8. Participates in interdisciplinary nursing rounds for inpatients (discharge planning process).
  9. Reinforces pre- and post-operative instructions and provides further education as appropriate.
  10. Maintains two-way communication with providers’ offices in order to improve the process of coordinating care for patients.
  11. Reports outcome of nursing/patient care/coordination activities to Cardiology Service Line leadership.
  12. Identifies patient and/or provider needs and develops quality programs to meet those needs. Ensures delivery of quality services for patients.
  13. Collaborates with outreach/education coordinators for all service lines in the preparation and delivery of community and professional education programs.
  14. Collaborates with research team in clinical trials enrollment and data collection.
  15. Serves as a resource to support groups and other activities.
  16. Revises and monitors educational materials to ensure that they are coordinated, understandable, and meet the PFEAT standards.
  17. Protects patient confidentiality by promoting professional staff communications.

Qualifications

  • Minimum Experience:
    • 3 - 5 years nursing experience
  • Minimum Education:
    • Bachelor's Degree
  • Required Certification:
    • Certification in appropriate nursing specialty; Cardiology/heart failure nursing experience preferred
