Care Coordinator (LPN) - Infusion Center

Job Description
Under general supervision, coordinates all aspects of treatment, evaluation, support services and education for patients. Coordinates clinical documentation from referring and treating physicians for each patient.
Duties and Responsibilities

Provides initial contact with patients.

Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment.

Provides individualized information, support and education.

Coordinates appointments for consultations and physician visits.
-
Notifies primary care and specialty care providers with any changes in treatment protocol.

Reinforces pre- and post-procedure instructions and provides further education as appropriate.

Maintains two-way communication with providers’ offices in order to improve the process of coordinating care for patients.

Reports outcome of nursing/patient care/coordination activities to Service Line leadership.

Serves as a resource to support groups and other activities.

Protects patient confidentiality by promoting professional staff communications.
Qualifications
Minimum Experience:
-
2-4 years Nursing experience
Minimum Education:
-
Certificate of completion (degree or diploma) from an accredited LPN program
Required Certification:
-
Current LPN license in PA; Basic Life Support (BLS) certification
