Job Description

Under general supervision, coordinates all aspects of treatment, evaluation, support services and education for palliative patients. Coordinates clinical documentation from referring and treating physicians for each patient.


Duties and Responsibilities

  1. Provides initial intake phone contact with palliative patients.
  2. Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment.
  3. Provides individualized information, support, and education regarding illness.
  4. Coordinates appointments for consultations and physician visits.
  5. Notifies primary care and specialty care providers when patient is admitted to the Hospital and/or significant changes in their condition.
  6. Follows patient via contact with inpatient nursing staff.
  7. Participates in interdisciplinary nursing rounds for inpatients (discharge planning process).
  8. Maintains two-way communication with providers’ offices in order to improve the process of coordinating care for palliative patients.
  9. Reports outcome of nursing/patient care/coordination activities to Referring Provider.
  10. Identifies patient and/or provider needs and develops quality programs to meet those needs. Ensures delivery of quality services for patients.
  11. Collaborates with outreach/education coordinators in the preparation and delivery of community and professional education programs.
  12. Protects patient confidentiality by promoting professional staff communications.
  13. Answer incoming calls and communicate with patients, care givers and palliative team.
  14. Coordinate prescription authorizations with pharmacies and patient’s insurance provider.


Nonessential Accountabilities:

  1. Participates in interdisciplinary conferences (as needed).
  2. Responsible for the care and maintenance of department/entity equipment and supplies.
  3. Attends meetings as required and participates on committees as requested.
  4. Performs other related duties as identified.

  1. Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
  2. Demonstrates knowledge of the patient’s illness and clinical course over the life span of the patient.
  3. Assesses and interprets patient age specific data and provides appropriate, age specific treatment.
  4. Provides direct patient care to assigned patient.
  5. Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
  6. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.




  • Notes:
    • Other combinations of formal education, training and experience may be considered.
  • Required Experience:
    • 2-3 years nursing experience
    • Case Management, Palliative and/or Hospice experience preferred
    • RN License required; BSN preferred
  • Physical Demands:
    • Mostly sedentary work
    • Occasional prolonged standing/walking
    • Occasionally lifts, positions, pushes and/or transfers patients
    • Occasional reaching, stooping, bending, kneeling, crouching
  • Skills:
    • Proven outstanding interpersonal skills; demonstrated ability to function as a team player; excellent communication skills
