For a bit of background: The transdisciplinary pain program is comprehensive, evidence-based chronic pain program. It involves team-based care consisting of a physician, physical therapist, psychologist, massage therapist, acupuncturist, dietitian, and nurse care manager. Patients are enrolled in a 12-month program and care is delivered all under one roof and through development of a single-treatment plan based on each patient’s individual needs. Initially, patients are seen weekly, then biweekly, then monthly over the course of 12 months. The boundaries between disciplines are intentionally blurred to ensure the patient receives a consistent message reinforced by every team member at every interaction.
Here is a link to a helpful video where Dr Toby, the CMO for Savas Health, presents an overview of the model. It also contains several patient testimonials at the very end. https://www.youtube.com/watch?v=yEU_LxUWP70.
