Provide healthcare to complex care patients with chronic pain in a healing environment aimed at restoring physical and emotional functioning and improving overall quality of life.

 

Position Highlights

  • Join a team in a clinically effective model to treat chronic pain, relieve suffering and improve function, decrease high-cost, avoidable care and lower opioid utilization for patients
  • Comprehensive, trauma-informed, team-delivered care where multiple disciplines provide care at one location based on a single treatment plan
  • Collaborate with the transdisciplinary team to develop, execute, monitor, and adjust comprehensive and coordinated, individualized care plans that include shared goals with measurable outcomes including cost of care.
  • Employ evidence-based approaches to shared decision-making that respect the patient’s preferences and social norms while targeting optimal treatment outcomes through patient empowerment

Duties and Responsibilities

  • Interviews patients and family members to obtain information about home environment, family relationships, health history, and patient's personality traits.
  • Conducts individual and group therapy sessions. Assesses patients' progress and modifies treatment program accordingly.
  • Provides therapeutic intervention with patient and family including as appropriate, individual, marital, parental, family and group techniques.
  • Documents current, accurate observations of care to the patient in appropriate records.
  • Coordinates care of assigned patients.
  • Assists in the implementation of education and training experience for learners from various disciplines.
  • Prepares and maintains written documentation as required by the profession and the department.

Required Experience/Education

  • Minimum Experience:
    • 6 - 12 months
  • Minimum Education:
    • Master's Degree in Social Work or Human Service related field
  • Minimum Field Of Expertise:
    • Social Work with Clinical specialty. Behavioral health counseling and CBT experience with the adult pain population preferred

For a bit of background: The transdisciplinary pain program is comprehensive, evidence-based chronic pain program.   It involves team-based care consisting of a physician, physical therapist, psychologist, massage therapist, acupuncturist, dietitian, and nurse care manager.  Patients are enrolled in a 12-month program and care is delivered all under one roof and through development of a single-treatment plan based on each patient’s individual needs.  Initially, patients are seen weekly, then biweekly, then monthly over the course of 12 months.  The boundaries between disciplines are intentionally blurred to ensure the patient receives a consistent message reinforced by every team member at every interaction.
