General Summary:

Under direct supervision of the Manager of Support Operations, provides clinical support services to patient care staff by performing a variety of duties related to patient care including assisting with intakes, groups, and patient assessments. Acts as a liaison between the clinical staff and patient/family. Provides brief therapy interventions, case management and community linkage and crisis management, both virtual and in person. Develops a good collaborative, professional relationship that enhances each of their respective functions and promotes improved outcomes and clinical efficiencies. Works in a team model to address patient transition and coordination needs. Has a basic understanding of behavioral health and physical health and will work closely with the outpatient staff and the WellSpan system of care.


Duties and Responsibilities:

1. Assists with various screenings, intake paperwork and treatment planning as appropriate.

2. Assist with facilitation of groups to include implementing therapeutic interventions and group documentation as necessary.

3. Verifies required documentation in patient record.

4. Schedules follow up appointments and completes follow up contacts between appointments.

5. Completes warm hand-offs to/from providers and assists with transitions between outpatient services and helps with care coordination.

6. Completes follow up contacts between appointments.

7. Send applicable correspondence when necessary.

8. Documents and records information into the patient record by entering, recording, storing, and maintaining information in written or electronic form.

9. Obtains or ensures accuracy of authorization for release of information forms as applicable.



Minimum Education:

(i) Has a bachelor’s degree from a college or university that is accredited by an agency recognized by the United States Department of Education or the CHEA in a recognized clinical discipline including social work, psychology, nursing, rehabilitation or activity therapies.

(ii) Has a graduate degree in a clinical discipline with 12 graduate-level credits in mental health or counseling from a program that is accredited by an agency recognized by the United States Department of Education or the CHEA.

(iii) Has an equivalent degree from a foreign college or university that has been evaluated by the AICE or the NACES. The Department will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency.


Minimum Experience: 1 year experience in a behavioral health-related office with specialized or technical skills/knowledge required.

Req Cert/Registration: BLS certification

Skills: Other Excellent human relations and oral/written communication skills; analytical abilities; computer and keyboard skills; knowledge of and ability in the care and handling of patients in all age groups; knowledge of PDMP, Lexicomp, Lippincott, Systems of Care, eCare, community services and/or other physician support systems; proficiency in Therapeutic Crisis Intervention (TCI) principles of de-escalation; Knowledge of EPIC; Knowledge of Insurance

Other Must possess a valid driver’s license in the current state of residence; driving record must meet requirements established by WellSpan Risk Management
