General Summary:
Under the direct supervision of the nurse manager/designee, and in concert with other team members, this staff works collaboratively to facilitate a stable and smooth operation of the unit and provides therapeutic interventions in accordance with best practice guidelines, care plans and policies.
Duties and Responsibilities:
The following are essential job accountabilities:
1. Coordinates and monitors the activities of the individual’s plan of care.
2. Performs all duties and responsibilities of the nursing assistant job description.
3. Provides direct care to patients primarily assisting with behavioral symptom management techniques and to assist patients in adapting to and/or coping with internal and external stresses.
4. Provides assistance to the care team in development of and updating the care plan and discharge planning.
5. Promotes patient attendance and participation in activities and documents participation.
6. Provides practical help and side-by-side coaching, problems solving, training, and assistance to help patients with their activities of daily living and concerns.
7. Provides direct care to patients in a manner that considers safety, comfort, and the therapeutic environment.
8. Assists the team members in the care and needs of the patients.
9. Assists with performance improvement measures and improving outcomes.
Qualifications:
Minimum Education High school diploma
Minimum Experience 1 – 2 years
Min Field of Expertise: Healthcare
Req Cert/Registration: CPR certification
Pref Cert/Registration: Nursing Assistant certification
Skills: Other Excellent human relations and oral/written communication skills; analytical skills, organization and time management; mental and visual ability to observe patient behavior and document findings; teaching ability; excellent customer service.
Other Knowledge of therapeutic activities, preferred
