Activity Specialist/Nursing Assistant

General Summary:
Under the direct supervision of the nurse manager/designee, and in concert with other team members, this staff works collaboratively to facilitate a stable and smooth operation of the unit and provides therapeutic interventions in accordance with best practice guidelines, care plans and policies.
Duties and Responsibilities:
The following are essential job accountabilities:
- Coordinates and monitors the activities of the individual’s plan of care.
- Performs all duties and responsibilities of the nursing assistant job description.
- Provides direct care to patients primarily assisting with behavioral symptom management techniques and to assist patients in adapting to and/or coping with internal and external stresses.
- Provides assistance to the care team in development of and updating the care plan and discharge planning.
- Promotes patient attendance and participation in activities and documents participation.
- Provides practical help and side-by-side coaching, problems solving, training, and assistance to help patients with their activities of daily living and concerns.
- Provides direct care to patients in a manner that considers safety, comfort, and the therapeutic environment.
- Assists the team members in the care and needs of the patients.
- Assists with performance improvement measures and improving outcomes.

- Demonstrates working as one with other team members, maintaining positive solution-focused communication, taking initiative, leading by example, demonstrating open and consistent communication, understanding of diagnosis and clinical interventions, and investing in the development of growth of staff and the unit.
- Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population.
- Establishes and maintains positive therapeutic relationships with the patients.
- Demonstrates commitment to patients, visitors and other staff by complying with all applicable safety regulations, identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety, and participating in real time problem solving, 4C and root cause analyses as required.
- Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
- Enhances professional growth and development through participation in educational programs, current literature, staff meetings and workshops.
- Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
- Maintains proficiency in principals of de-escalation.
- Completes documentation according to established standards.
The following are nonessential job accountabilities for ADA purposes:
- Attends scheduled meetings as required.
Performs other related duties as identified.
Qualifications:
Note: Other combinations of formal education, training and experience may be considered.
Minimum Education High school diploma
Minimum Experience 1 – 2 years
Min Field of Expertise Healthcare
Req Cert/Registration CPR certification
Pref Cert/Registration Nursing Assistant certification
Working Conditions Clinical and normal office environment
Occasional exposure to personal harm/injury
Skills: Other Excellent human relations and oral/written communication skills; analytical skills, organization and time management; mental and visual ability to observe patient behavior and document findings; teaching ability; excellent customer service.
Other Knowledge of therapeutic activities, preferred
